Report of A Case of Appendico-jejunal Fistula

Abstract
Appendico-jejunal fistula is a rare entity that has seldom been reported in the literature, so we are presenting a case, recently at "our hospital, and a review of the literature.

Appendico-jejunal fistula have been reported only 2 times in the american literature by Walker et al (1969) and Arlen (1970) and these 2 cases were both discovered at surgery.

Our case was operated on because of small bowel obstruction. The point of obstruction was just proximal to the fistula where there was a constricting band. The etiology was obscure and no stones were discovered in the fistula and no biliary stones were detected by palpation during the operation. The presence of adhesion in the area suggests a previous inflammatory disease of the intestine. Prior acute appendicitis was probably not the original cause of the fistula because the appendix was thin wall and distended rather than indurated.

The fistula resulted in a bypass of the ileum which was empty and atropic in appearence. The meager body weight of the patient suggests nutrient loss due to the bypass.

At surgery the fistula was interrupted and the obstruction corrected so that enteric contents .would be able to pass through, the full lenght of the small intestine. Recovery in this case was uneventful. Follow up 8 weeks later revealed a weight gain of 1.3 kg. in dicating improved intestinal absorption.
